Please complete and submit a form for any guests. Tickets for the events will be distributed based on receipt of this form. The form produces a ticket and that ticket will be your admission to the event.

The $30.00 CS4 membership fee may be pre-paid to NCSS(with your NCSS membership, CS4 (care of John LeFeber), or it will be collected from each participant at the meeting.

Please remember that a member of CS4 must also register for membership to the
National Council for the Social Studies (NCSS)
